ABSTRACT
Objective To explore the relationship between the level of serum Klotho with renal impairment in chronic kidney disease (CKD) patients. Methods 100 patients with CKD 1-5 stages (CKD group) and 30 non-CKD patients (control group) were enrolled. The serum of Klotho was detected by ELISA (enzyme-linked immunosorbent assay ) to investigate the correlation between the level of serum Klotho to progression of CKD. Results The levels of serum Klotho in CKD group 9.81 (7.45,12.2) ng/mL and control group 2.97 (1.75,5.23) ng/mL were significantly different (P < 0.01); The Klotho level declined as renal insufficiency progressed. Pearson correlation analysis showed that serum Klotho in CKD patients was positively correlated with eGFR (estimated glomerular filtration rate), blood calcium and hemoglobin,while was negatively correlated with phosphate, age, logserum creatinine, and logFGF-23. Multiple regression analysis showed age, logScr and logFGF-23 were independent factors for expression of serum Klotho in CKD patients. Conclusion Serum Klotho level in CKD patients decreased with the deterioration in renal function and serum Klotho maybe one of indicators to predict the progression of renal function in CKD patients.
ABSTRACT
Objective To compare the short-term therapeutic effect, adverse reaction and influence on immune function between chrono-chemotherapy and routine-chemotherapy with docetaxel +cisplatin(DP)combined with concurrent chemoradiotherapy with cisplatin(DDP)in locally advanced(Ⅲ,Ⅳa stage)nasopharyngeal carcinoma(NPC).Methods 70 cases of newly diagnosed locally advanced NPC were randomly divided into group A and group B,35 cases in each group.Two groups of patients were treated with two cycles of neoadjuvant chemotherapy with DP combined with concurrent chemoradiotherapy with DDP.Both nereoadjuvant chemotherapy and concurrent chemotherapy of patients in group A were administrated with chrono-chemotherapy.Both nereoadjuvant chemotherapy and concurrent chemotherapy of patients in group B were administrated with routine-chemotherapy.Therapeutic effect,adverse reactions and other indicators were observed in both groups.Results After 3 months,there was no sifnificant difference of short term total efficacy between two groups in nasopharyngeal primary tumor(35 cases vs.34 cases),cervical lymph node(32 cases vs.31 cases).The incidence of adverse reactions of marrow toxicity(WBC,Hb,PLT)in group A was lower than group B(P<0.05),the incidence of digestive tract(nausea and vomiting,diarrhea)in group A was lower than group B(P<0.05),while there was no significant difference in oral mucositis between two groups(P<0.05).The high creatinine incidence in group A was lower than group B(P<0.05).There was no significant difference in high blood urea nitrogen and high transaminase incidences between two groups.After treatment, the T lymphocytes ( CD3 +, CD4 +, CD8 +, CD4 +/CD8 +) had no change in group A,the T lymphocytes(CD3 +,CD4 +)in group B decreased significantly compared with pre-treatment(P<0.05),and were lower than group A(P<0.05).Conclusion Adverse reactions and the degree of decline in cellular immune function of chrono-chemotherapy are lower than routine-chemotherapy in locally advanced NPC.

Objective To compare the clinical efficacy between single span video-assisted thoracoscope (VATS) surgery and traditional VATS surgery for spontaneous pneumothorax in elderly patients.Methods A total of 88 elderly patients diagnosed as spontaneous pneumothorax by chest X-ray and CT were selected and randomly divided into single span VATS group and traditional VATS group (n=44,each).The bleeding volume,duration of operation,hospitalization time,time of postoperative analgesic use,chest tube indwelling time,postoperative complications and adverse reactions were observed.All patients were follow-up for 1-2 years to record the recurrence rate and death rate.Results All patients were received VATS surgery successfully.Their clinical symptoms disappeared and X-ray showed that pneumothorax disappeared.There were significant differences in bleeding volume,time of postoperative analgesic use,hospitalization time between single span VATS group and traditional VATS group [(46.7±18.8)ml vs.(46.7±18.8)ml,(1.7±0.9)d vs.(2.4± 1.7)d,(6.3±1.7)d vs.(9.1±2.1)d,t=3.04,2.18,5.59,respectively,all P＜0.05].The duration of operation and chest tube indwelling were shorter in single span VATS group than in traditional VATS group (both P＜0.05).Patients with chest pain and incision numb after operation were less in single span VATS group than in traditional VATS group (11 cases vs.26 cases,7 cases vs.21 cases,t=10.50,10.26,both P＜0.05).There were no significant differences in recurrence rate and death rate between the two groups.Conclusions Single span VATS surgery is an safe and effective treatment for spontaneous pneumothorax in elderly patients,which has a better postoperative recovery and fewer adverse reactions.
